Job Summary:
Orlando Treatment Solutions is seeking a dedicated Case Manager to join our team. As a Case Manager, you will play a crucial role in providing comprehensive care and support to individuals in need of PHP and IOP services. You will work closely with clients, healthcare providers, and community resources to ensure the delivery of high-quality care and effective treatment plans.

Responsibilities:

- Provide case-management services including coordinating care and discharge planning to determine client needs and help develop individualized care plans.
- Coordinate and monitor client progress throughout their treatment journey.
- Work with the Orlando treatment team, admissions team, alumni coordinator to optimize client care.
- Provide support services to clients in our PHP and IOP services

-Run PHP groups as needed
- Maintain accurate and up-to-date EMR documentation related to client care.
- Work with diverse populations including those with both mental health and substance use needs
- Advocate for client rights and access to necessary mental health services.
- Participate in team meetings, and training sessions to enhance professional development.

Qualifications:
- High School diploma, Peer Support Specialist, Case Management Certification or Bachelor's degree in related field.
- Proven experience in case management, preferably in a mental health or healthcare setting.
- Ability to work effectively in a fast-paced clinical setting while maintaining a high level of compassion and empathy for clients.
- Excellent communication skills and the ability to collaborate with multidisciplinary teams.
- Proficiency in maintaining detailed medical records and using electronic medical record systems.
- Understanding of PHP and IOP mental health and substance use services.

Pay: From $45,000.00 per year
