The Centre of Hope invites applications for: 1 NEAFAN FASD Outreach Coordinator - Temporary Full Time
Note: Perquisite for this positing requires applicants to have a minimum 2-year social sector diploma and 2-year related experience working with at risk populations. Along with 1 sample of case note documentation.
As a support staff you will be required to:
Providing support, advocacy and resource brokering to individuals referred to outreach services.
Developing community connections and ensuring communities in the service area are familiar with the process of referral and have knowledge of the FASD services.
Completing administrative duties.
Accessing and connecting with community services
Developing relationships with children/youth and their caregivers/support network.
Assist in development and maintenance of consistent referral processes, intake information and records
Provide direct case management to individuals diagnosed or suspected
Work collaboratively with community resources.
Maintaining individual client files, record keeping, navigating program database, reporting, and all other forms of reporting as per contract requirements.
Adhere to all policies and procedures of the Agency as well as Program structure.
Qualifications:
A desire to make a difference in the life of individuals suspected of or diagnosed with FASD.
Minimum 2-year diploma
Minimum 2 years working experience in related field.
Minimum 1 year experience working in amongst the vulnerable population as well as previous experience with the FASD program.
A reliable vehicle, valid driver’s license, registration, and insurance ($2 000 000 liability insurance).
Strong organizational and time management skills.
Strong verbal and written communication skills.
Ability to work independently as well as in a team environment.
Ability to be flexible and adapt to change.
A successful candidate will be required to provide a Police Information Check and Child Intervention Record check.
Demonstrated effective communication skills.
Nonviolent crisis intervention training.
Ability to foster partnerships, establish and maintain strong working relationships with public agencies and school boards.
Advanced knowledge of issues relating to activities of street involved people.
Extensive knowledge of resources and services available within the community specific to FASD.
