About the Role

A vacancy has arisen in the disability support link team for a Needs Assessor and Service Coordinator working with clients aged 65 and under with a physical, intellectual, Autistic Spectrum Disorder or sensory disability.

About the Service

Disability support link is the needs assessment and service coordination agency for Waikato DHB. We are comprised of two teams; the Health of Older People team and the Disability Support Services team.

Our team cover a range of contracted services for people under the age of 65 with an intellectual, physical, or sensory disability, and autistic spectrum disorder.

About you

The successful candidate will ideally be a registered health professional, though this is not essential, and have experience working with people with disabilities and their whanau. Previous experience in needs assessment and service coordination is desirable.

  • have excellent communication skills
  • enjoy working in partnership with people of all ages and their families to develop an individualised support package
  • enjoy working with a multidisciplinary team and across all sectors
  • have previous experience working in the disability sector
  • have an understanding of the Treaty of Waitangi
  • have a current full New Zealand drivers licence
