About the role:
The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)
is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

The Nurse Practitioner (NP) in the Surgical Transitions Remote Patient Monitoring Program is an autonomous,
advanced practice nurse who provides comprehensive, collaborative care to a defined patient population

The NP will support and navigate the patient's surgical journey. The NP will be the patient advocate and provide care
in a highly supportive and personal manner, and serve as a liaison to ensure that patients and families receive
resources and services as needed.
The NP reports primarily to the Manager of the Regional Surgical Services Program in collaboration with the Director
of Regional Surgical Services, Director of Surgical and Ambulatory Care services, with a matrix reporting relationship
to the Director, Nursing and Health Professions, The role of the NP integrates the six core competencies of
Advanced Practice Nursing as outlined by the Canadian Nurses Association: direct comprehensive care, health
system optimization, education, research, leadership, and consultation and collaboration. The percentage of time
dedicated to each of these domains is variable, based on the program needs and will evolve with the program.

EMPLOYMENT REQUIREMENTS:
Education/Experience:

*Unencumbered registration as a Registered Nurse in the Extended Class (EC) with the College of Nurses of Ontario
*Membership in NPAO or RNAO preferred
*Minimum 3 years recent nurse practitioner experience in an acute care setting during past 5 year period.
*Demonstrated commitment to the principles of the Patient and Family Centred Care model.
*Clinical knowledge and expertise in surgical care.
*Knowledge of the research process and ability to apply research to clinical practice.
*Previous adult education and program development, implementation and evaluation experience.
*Knowledge of local and regional issues related to acute and chronic pain and rehabilitation.

*Proficient in computer applications and software such as, Microsoft Office, Powerpoint and Excel.
Salary Range: $68.680 to 76.140
