BASIC FUNCTION

The Nurse Practitioner will represent Wellington Terrace by ensuring adherence to the Code of Conduct and demonstrating the County of Wellington’s commitment to a healthy and safe workplace where every person is respected.


Reporting to the Director of Care, the Nurse Practitioner is a primary care provider to residents and works within the legislative scope of practice as described and outlined by the College of Nurses of Ontario. The Nurse Practitioner collaborates with the resident and family/care giver, along with our interdisciplinary care team in the development, implementation, and evaluation of our residents’ plan of care; provides leadership and mentorship to other team members; and leads and collaborates research, education, and evidence-based practice initiatives

PRINCIPAL RESPONSIBILITIES

Direct Care Services

  • Provide comprehensive primary care to residents as part of the interdisciplinary health care team.
  • Perform a person-centered health assessment of residents on admission, annually and as needed (including quarterly medication reviews).
  • Prescribes pharmacological and non-pharmacological therapies according to approved medical directives.
  • Performs skills and invasive/non-invasive procedures related to specialty.
  • Increase continuity of care through collaboration, consultation, and referral as appropriate.
  • Meets quarterly with Administrator, Assistant Administrator, Director of Care and Medical Director.


Leadership

  • Participate in creating an organizational environment that supports the safety quality of resident care and life, collaborative practice, and professional growth.
  • Provide leadership and involves the interdisciplinary team in quality improvement initiatives.
  • Participates and provides leadership in the strategic planning process for the clinical programme(s).
  • Provide leadership in developing and implementing strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities.


Teaching and Coaching

  • Provide formal and informal teaching and coaching in the management of clinical care to interdisciplinary team members, serving as resource person, educator and role model, and contributes to the performance appraisals of registered nursing staff.
  • Contribute to planning, implementing and evaluating learning resources and health education programmes for residents, families and substitute decision makers.
  • Analyzes and interprets current research / literature / knowledge applicable to health care practice with the identified client population.

MINIMUM QUALIFICATIONS

  • Four year degree in Nursing, combined with a two year Primary Health Care Nurse Practitioner graduate programme.
  • Minimum three years of experience or equivalent working in a health care setting, preferably as a Primary Health or Long Term Care Nurse Practitioner.
  • Must possess current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (EC).
  • Current certification in Basic Cardiac Life Support.
  • Education or experience in the areas of dementia and palliative care is an asset.
  • Experience working in a leadership capacity in a health care setting is an asset.
  • Demonstrated experience in gerontology preferred.
  • Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management.
  • Knowledgeable about primary health care issues in Long Term Care.

Pay Range: $127,017.80 - $148,603.00 (2026 Non-Union Compensation Grid).
Benefits: Long Term Disability, Life Coverage, Accidental Death and Dismemberment, Critical Illness, Extended Health Care, Emergency Travel Assistance, Dental Care; mandatory enrolment, premiums 100% covered.
Pension: Ontario Municipal Employees Retirement System (OMERS); enrolment mandatory.
Medical Responsibility Time: Prorated and accrues at 0.5 days/month. Short Term Disability: 17 weeks coverage at 75% or 100% of pay, based on length of service.
Vacation: Accrues to start at three weeks/annum; increases based on length of service.
