Job Summary
The Parent-Child Assistance Program (PCAP) is a paraprofessional intervention for high- risk women and their families. This position will be responsible for providing direct outreach, home visitation, case management, and advocacy services to high-risk mothers who have used alcohol and/ or drugs heavily during pregnancy, and their children, partners, and extended families when needed.
Duties and Responsibilities
- Build and maintain a trusting relationship with women and their families in order to optimize parent/child relationships.
- Perform case management activities according to the PCAP model, in a highly confidential and professional manner.
- Manage case load of 8-16 clients and their families.
- Conduct home visitation
- Assist individuals in setting goals based on their individual need’s assessments, and in identifying steps to achieve their goals: monitor progress regularity.
- Provide outreach services to engage and connect individuals with community agencies that will help meet their needs and goals.
- Collect and enter data online
- Take a harm- reduction, client focused and strength-based in approach in home visitation, outreach and case management activities
- Complete individual intake and assessment, case planning and data entry.
- Provide supported referrals to appropriate community agencies Provide case management supports including monitoring, mentoring and service coordination
- Provide advocacy and support around health care, education, employment, housing, education and/ or criminal or family justice issues and additional matter that may aris
- Network with and maintain positive relationships with other service organizations
- Educate individuals and their families in understanding the effects of prenatal alcohol exposure
- Serve as a positive role model for clients, providing life skills training, and behaviour management; supporting women and their families in the implementation and practice of life skills and positive coping strategies
- As necessary, provides transportation and assists in attending appointments
- Support parents/caregivers to improve the family support system, increase the family’s ability to problem solve and assume the role of advocate for themselves
- Identify and maintain knowledge of outside referral sources and providers
- Ensure that required documentation is completed in within time frames.
- Attend and participate, if required, in the diagnostic process
- Work cooperative and closely with the NW Peace FASD Society
- Attend staff meetings, supervision and training as required
- As well as all other duties requested by Executive Director
Qualifications:
- Must be able to work in a fast paced envirnment
- Bachelor or Diploma in Social Work, addictions or related field or combination of relevant education and minimum of two years experience.
- Understanding of addictions, mental health, and harm reduction philosophy and practice
- Proven organizational and time management skills.
- Proven written and verbal communication skills, problem solving skills, and ability to work with a wide range of clients.
- Proficiency in Windows environment, Office 365, and operation of office equipment.
- Vehicle and valid driver’s license.
- All required training such as but not limited to, first aide, FOIP, PCAP training Etc.

Job Type: Full-time
Pay: $27.00-$28.00 per hour
