Patient Care Coordinator (Pcc)

Sorin Medical, P.C., the leading interventional cardiology practice in New York City, is seeking a skilled and experienced Patient Care Coordinator.

Position: Patient Care Coordinator

Key Responsibilities:

• Patient Care Coordination: Plan, coordinate, and implement the care of valve patients in the outpatient setting.
• Testing Coordination: Coordinate any additional testing required for patient care.
• Procedure Scheduling: Facilitate the scheduling and pre-authorization of TAVR, Watchman, and Mitral Clip procedures.
• Clinic Operations: Coordinate the implementation and operations of the heart valve clinic and valve program.
• Collaboration: Work closely with physicians, departments, staff, and patients to achieve the goals of the Structural Heart Program.
• Patient Navigation: Navigate patients through all phases of their care to optimize outcomes.
• Diagnostic Testing: Ensure proper and complete scheduling of patient diagnostic testing following established protocols.
• Organizational Management: Maintain efficient organizational management of patient care, including timely scheduling of preoperative and postoperative care.
• Detail-Oriented Management: Focus on the overall management of patient care, ensuring accuracy and thorough documentation in patient medical records.
• Referral Management: Maintain a referral work queue for scheduling referrals to the Structural Heart Clinic, ensuring accurate entry of patient information for robust tracking and data collection.
• Echo Report Management: Pull echo reports and screen echo data to manage patients with valvular heart disease.

Qualifications:

• Strong organizational and time management skills.
• Detail-oriented with a focus on thorough and accurate documentation.
• Excellent communication and interpersonal skills for effective collaboration with physicians, departments, staff, and patients.
• Experience in navigating and managing multiple phases of patient care to ensure optimal outcomes.
