DUTIES AND RESPONSIBILITIES:
The Patient Care Manager (PCM), Medicine is responsible for the management of the clinical service area and providing visible leadership to the interdisciplinary team to ensure optimal performance management and smooth operation of the service area and high quality patient care. As a transformational leader, the PCM will create a vision to guide positive change and innovation through inspiration, motivation, role modelling and coaching and by working in collaboration with the leadership and interprofessional teams in the Medicine Program. The PCM facilitates and supports implementation of evidence based practice to achieve excellence in high quality delivery of patient care.
The Patient Care Manager, Medicine also works closely with the Program Director, Patient Care Managers and Clinical Practice Leaders, to lead and guide specific key quality improvement, strategic and performance improvement initiatives identified as priorities in the Medicine Program. Integrating strategy, quality improvement, practice and project planning across all services within the Medicine program to achieve deliverables and excellence in patient care is a key component of this role. The PCM also contributes to the implementation of continuous improvement in the program to achieve excellence in person-centered care through the utilization of skills and attributes associated with the fields of process improvement, project management and change management in an integrated manner.
The Patient Care Manager will also oversee the Medicine Float Pool team and lead initiatives to optimize the functioning of the team and processes, including implementing an effective team structure, designing and implementing optimized master schedules and proactive staffing practices that meet operational needs, strategic workforce planning and aligning scheduling technologies and processes.

QUALIFICATIONS:
- Undergraduate degree in a health related discipline (Masters preferred);
- Minimum of two (2) years recent clinical/leadership experience;
- Minimum of two (2) years' experience in quality improvement development, implementation and evaluation;
- Current member in related professional interest group or association preferred;
- Educational preparation in quality improvement, certificate in quality improvement or equivalent in the use of quality improvement methods and tools;
- Project Management skills and experience
- Related knowledge in related clinical specialty;
- Knowledgeable in the principles of adult education and teaching/learning principles.
- Knowledge of current trends in nursing, allied health and health care as well as applicable legislative and hospital/program policies;
- Demonstrated knowledge of quality improvement principles, methods, and tools with excellent project management skills including assessment, planning, implementation and evaluation skills at a program and corporate level
- Demonstrates excellent verbal and written communication.
- Strong analytical skills including the ability to collect, analyze, share and interpret qualitative and quantitative data
- Ability to use Microsoft Office applications such as Word, Excel, Outlook, PowerPoint, Project, and SharePoint and healthcare related software and programs.
