We are currently recruiting for full-time Patient Flow Coordinator for our Beaumont Lodge Transitional Car Unit, in Artane, Dublin 5.
Purpose of The Role:
- To support the multidisciplinary team by scheduling, daily admissions, and coordination of bed management at Beaumont Lodge Transitional Care Unit, and to discharge bed management function in accordance with Bartra Healthcare Standards of Quality Care.
Roles and Responsibilities:
Bed Management Responsibilities:
- To Develop rapport with critical stakeholders.
- To Ensure that referral, admissions and discharge process is adhered to at all times.
- To Conduct a thorough clinical assessment of each patient referrals either on site or via telephonic assessment as appropriate.
- To Allocate patient to the most appropriate ward/bed available to assure safe standard of nursing care.
- To Monitor bed occupancy and send daily reports to senior management.
General Responsibility
- The person holding this post is required to support the principle that the care of the patient always comes first and will approach their work with the flexibility and enthusiasm necessary to make this principle a reality for every patient to the greatest possible degree.
- Ensure that the provision of a high standard of care to the patient or patients is consistent with the mission, vision, values and strategic plan of Bartra Healthcare
Management Responsibilities
- Be responsible for the following in relation to Governance, Leadership and Management:
- In the absence of the Assistant Director of Nursing, undertake the role and responsibilities of the Person-in-Charge in line with the Health Act 2007, related regulations and the Health Information and Quality Authority (HIQA) standards.
- Support the Registered Provider/Director of Nursing the day to day management of the Place of Work.
- Act as a member of the Bartra Healthcare Teams, including, as deemed required by the Registered Provider/Director of Nursing, the Multidisciplinary Care Team, the Risk Management Team, the Incident Management Team and the Infection Control Team.
Delegation of Tasks and Duties
- Ensure that where directing delegation of care, the delegation is appropriate to the definitions and philosophies of nursing. Delegation shall take into account the levels of experience, competence, role and scope of practice of the staff member taking on the delegated task.
- Be accountable for roles and activities delegated to junior staff, where appropriate.
- Work towards a restraint-free environment, with implementation of practices in accordance with the National Policy.
Quality and Safety Management System:
- Support and implement the Quality and Safety Management System, including resident involvement. The Quality and Safety Management System shall encompass ongoing monitoring of the service and care provided within Bartra Healthcare Facility, including the annual review of the quality and safety of care provided.
- Participate in the Bartra Healthcare processes as deemed required by the Registered Provider/Director of Nursing. This may include involvement and implementation of controls in the development and update of Bartra Healthcare Risk Management Registers and resident’s Individual Risk Management Plans.
- Recommend the development and/or revision of policies and procedures to the Registered Provider/Board of Directors in line with applicable laws, standards and clinical best practice and be responsible for the development, implementation and compliance to the policies and procedures
Qualifications
- A registered General Nurse or Psychiatric Nurse on the Register of Nurses held by Nursing and Midwifery Board of Ireland or be entitled to be so registered.
- Evidence of continuous professional development.
Desirable:
- Post Graduate Diploma in Gerontology
- Management qualifications
- Gerontology experience
- Experience in Psycho Geriatric Nursing Care/Psychiatry/Adult Disabled.
- Have an in-depth knowledge of the ageing process.
- IT Skills
Experience
Essential:
- Minimum three years post graduate experience with a minimum of one-year experience at CNM level.
- Have an in-depth knowledge of the ageing process.
Desirable:
- Experience in psycho geriatric nursing care/psychiatry/adult disabled.

- Knowledge of Residential Care legislation and standards including:
- The Health Act 2007 (Number 23 of 2007)
- S.I. 415 of 2013 Health Act 2007 (Care and Welfare of Residents in Designated Centres for Older People) Regulations 2013)
- S.I. 293 of 2016 Health Act 2007 (Care and Welfare of residents in designated centres for older people (Amendment) Regulations 2016. 4
- S.I. 61 of 2016 Health Act 2007 (Registration of Designated Centres for Older People) Regulation 2015.
- HIQA’s National Standards for Residential Care Settings for Older People in Ireland (2016).
- HIQA’s National Standards for the Prevention and Control of Healthcare Associated Infection (2009)
- HIQA National Standards for Safer Better Healthcare (2012)
- Joint Commission International Accreditation Standards for Long Term Care (2012)
- Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (2014)
- NMBI Scope of Nursing and Midwifery Practice Framework (2015)
- Demonstrate sound knowledge and clinical nursing practice together with future trends in Care of Older Persons.
- Show evidence of current legislative policies pertinent to Care of the Older Persons.
Essential Knowledge:
- Knowledge and competence to practice effectively through Continuing Professional Development (CPD), following registration.
- Data Protection Act 2003
- Safety, Health and Welfare at Work Act 2005

- Nurses and Midwives Act 2011
- Educated in or familiar with the concepts and methods of quality improvement.
- Knowledge and understanding of the scope of practice for all Health and Social Care Professionals.
- Knowledge of relevant guidance:
- Protecting Our Future – Report of the Working Group on Elder Abuse (2002).
- Trust in Care. Policy for Health Service Employers on Upholding the Dignity and Welfare of Patient/Clients and the Procedure for Managing Allegations of Abuse against Staff Members (2005).
Desirable:
- Knowledge of organisation of health services and trends nationally and internationally.
- Professionalism in the application of theory to daily practices.
- Critical thinking to be able to recognise resident problems and adapt to changes quickly.
- Ability to recognise shortfalls or limitations of their competencies.
- Ability to source both theoretical and technical information.
