The Proactive Care Team Care Coordinator role helps with the coordination of care for individual residents with mild to moderate frailty and multiple long term conditions.
The post is a key role, supporting with identification and management of a caseload of suitable residents. It requires sensitive and skilled engagement to deliver support which develops residents ability to self-manage their health conditions and enables wellbeing and ageing well.
It helps to improve the access, experience and outcomes for people and reduce health inequalities. It includes liaison with a range of health, social care and voluntary organisations as well as active contributions to Multi-Disciplinary Team (MDT) Meetings on a regular basis
The role will directly support the delivery of Proactive Care and Personalised Care in City and Hackney. In particular the focus will be about supporting people with multiple Long Term Conditions and Frailty in the community who would benefit from proactive and coordinated care.
These will often be residents who have a range of physical, psychological and social support needs. Residents will, in some cases, be known to existing community-based services across City and Hackney but could be better coordinated across a multi-agency team. Some people might not be known to services currently and could benefit from a holistic, local offer that you will be part of.
The Care Coordinator will be a critical part of the Neighbourhood Team. The role will provide the ‘glue’ to bring together different professionals across City and Hackney – both members of the core Neighbourhood team but also wider services and voluntary and community sector as required.
The role is patient facing and you will be expected to make proactive contact with residents. At the core of this approach will be person-centred discussions with a focus on developing trusting relationships and showing compassionate curiosity to ensure all involved understand what matters to people and what is required to enable people to live well.
