Single Assessment Service (SAS) as part of the North West Melbourne Aged Care Assessment Alliance

The Single Aged Care Assessment System (SAS) has been set up by the Health Department to simplify and improve access to aged care services. The SAS includes assessments for all aged care services, including Aged Care Assessments and Residential Aged Care (RAC) Funding Assessments.

This position is responsible for completing RAC Funding Assessments at Australian Government funded aged care homes. This position will also work alongside the Clinical Aged Care Assessors to provide care needs assessments for people that are 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) who require assistance with everyday tasks. In doing this, the assessor will consider the restorative, physical, medical, physiological, cultural and social dimensions of clients care needs.

This is a Fixed Term position until 30 June 2027 (Funding Based Activity Project). There is flexibility to consider Full Time or Part Time, with a minimum commitment of 0.6FTE (3 days). The role is classified under either awards, depending on your qualifications and experience -

  • Community Nursing 4 offering $107,390 under the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024-2028; or
  • Allied Health Professional Grade 2 offering $92,874 - $106,801 under the Victorian Stand Alone Community Health Centres Allied Health Professionals Enterprise Agreement 2021-2022

You will make a difference by

  • Managing an assessment caseload and conduct Residential Aged Care (RAC) assessments
  • Deliver RAC Funding Assessments at Australian Government funded aged care homes, applying the AN-ACC Tool via My Aged Care to complete assessments that facilitate improved access to aged care services.
  • Empower clients by encouraging their active participation ensuring services are tailored to meet individual needs.
  • Maintain compliance with statutory and organisational standards by ensuring accurate client data, upholding quality care standards and meeting regulatory requirements.
  • Commit to partnering with Residential Aged Care facilities to facilitate effective client engagement and participation.

To succeed you will need:

  • Tertiary qualifications in Nursing, Occupational Therapy or Physiotherapy with current and unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Minimum of 5 years demonstrated experience delivering services in aged care setting and/or to aged persons ie geriatric evaluation, rehabilitation, palliative care, community nursing, people living with dementia.
  • Strong ability to conduct comprehensive, holistic client assessments and developing complex, individualised care plans that optimise independence and wellbeing, ensuring the highest standards of care and client-centred outcomes.
  • Maintain client data and record accuracy and integrity, including the My Aged Care portal
  • Strong communication skills, to effectively collaborate, engage with stakeholders and work within a multidisciplinary environment.
  • Ability to identify, assess and manage risks proactively, ensuring compliance with applicable standards, regulations, and guidelines.
  • Current and valid Victorian driver’s licence and Working with Children's Check.
