Position Summary

The RN Case Manager is responsible for coordinating and managing the overall plan of care for home health patients. This role includes performing comprehensive assessments, developing individualized care plans, coordinating services, supervising home health aides and LPNs, collaborating with physicians, and ensuring compliance with Medicare Conditions of Participation and agency policies.

Responsibilities

• Complete Start of Care, Recertification, Resumption of Care, and Discharge OASIS assessments.
• Develop, implement, and update individualized patient care plans.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Perform skilled nursing visits according to physician orders.
• Educate patients and caregivers regarding disease management and medications.
• Supervise and coordinate care provided by LPNs and Home Health Aides.
• Complete documentation accurately and within required timeframes.
• Participate in on-call weekend rotation as scheduled.
• Ensure compliance with Medicare, state, and agency regulations.

Qualifications

• Current Ohio Registered Nurse (RN) license.
• Minimum of one year of nursing experience (home health experience preferred).
• OASIS experience preferred.
• Strong assessment, communication, and organizational skills.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently while collaborating with an interdisciplinary team.

Weekend Rotation

This position includes participation in a weekend rotation to ensure continuity of quality patient care.
