Job Summary:
Are you a compassionate Registered Nurse seeking a clinical role where you can provide truly person-centered care? At PACE-RI, we empower older adults to live independently through a comprehensive, holistic medical model. We are searching for a dedicated RN to join our interdisciplinary team—a collaborative environment where your clinical expertise and heart directly impact participant outcomes. In this role, you will conduct comprehensive nursing assessments, develop individualized plans of care, and provide direct clinical intervention while supporting participants in managing chronic conditions and navigating end-of-life care. You will also serve as a clinical educator for families, triage urgent needs, and make critical clinical decisions in real-time.
As a vital coordinator of care, you will ensure seamless transitions by reconciling medications, arranging home health services, and maintaining meticulous continuity of care. This position requires high-level clinical coordination, including managing complex documentation, updating care plans, and facilitating clear communication between families and the interdisciplinary team. In partnership with our Home Care team, you will oversee clinical services delivered by CNAs, conduct in-home observations, and provide ongoing staff education to maintain our high standards of excellence. This role offers professional stability through a predictable rotating schedule for weekend and holiday coverage; typically occurring only once every 5 to 6 weeks, allowing for an excellent work-life balance.
