Resident Care Coordinator

Cedar Sinai Park is looking for a full-time Resident Care Coordinator!

This is a full-time, benefited position at our beautiful Rose Schnitzer Manor.

We are located at 6140 SW Boundary St., in the Raleigh Hills area of southwest Portland. Our campus is on bus routes 54 and 56 from Portland to Beaverton.

Summary- The primary purpose of this position is providing assistance to the Health Services Director in day-to-day care of residents and in meeting regulatory compliance with policies and procedures.

• Communicates with residents to determine satisfaction with care.

• Completes initial assessment of prospective residents.

• Coordinates and develops care plan of new residents.

• Coordinates care of residents, case management for residents and families.

• Participates and leads service plan meetings with residents and families.

• Understands policies, procedures and protocols related to medication management and emergencies.

• On call responsibilities.

• Strong organizational and time-management skills. Ability to effectively delegate to others.

• High degree of interpersonal relations skills and capability of relating to a variety of people and personalities.

• Able to assist the Director of Health Services with management of the Resident Assistants and Health Care Coordinators.

• Considerable initiative, judgment, and leadership skills are also required.

• Demonstrated computer proficiency.
