Overview
We are seeking a dedicated and compassionate Case Manager to join our team. This role is crucial for coordinating patient care, ensuring effective discharge planning, and facilitating access to necessary resources. The Case Manager will work collaboratively with healthcare professionals to provide comprehensive support to patients throughout their care journey.
Responsibilities
- Conduct thorough patient assessments to determine individual needs and develop personalized care plans.
- Collaborate with multidisciplinary teams.
- Facilitate discharge planning by coordinating follow-up appointments and community resources to ensure continuity of care.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Provide education and support to patients and their families regarding treatment options, resources, and health management strategies.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Assist with case management services for patients who are veterans.
Experience
Candidates should possess the following skills and experience:
- Associate's Degree.
- Knowledge of medical terminology and case management principles.
- Strong communication skills.
