A well-established and rapidly expanding Certified Home Health Agency (CHHA) in Brooklyn is seeking a skilled and detail-driven RN Case Manager to join its team. This agency is known for its warm culture, organized clinical structure, and consistent growth across the five boroughs.

What You’ll Do

• Manage a patient caseload and coordinate all aspects of CHHA care
• Develop, update, and oversee individualized care plans
• Communicate regularly with physicians, therapists, and interdisciplinary teams
• Review documentation for accuracy and compliance with DOH/Medicare and CHHA standards
• Collaborate with intake, authorizations, scheduling, and HHA departments
• Monitor visit frequencies, authorizations, quality outcomes, and timely billing
• Provide clinical guidance and support to field nurses when needed
• Participate in meetings such as QA/UR/PIP as part of the clinical operations process

What We’re Looking For

• Registered Nurse (RN) – NY license required
• Strong knowledge of CHHA operations, workflows, and documentation standards
• Proficiency with EMR/EHR systems and comfort navigating fast-moving caseloads
• Excellent communication, follow-through, and clinical decision-making
• A nurse who thrives in a well-organized environment with clear expectations and dependable support
