Daughters of Sarah is a 210-bed skilled nursing and rehab facility conveniently located on Washington Ave. Extension in Albany (within close proximity to Crossgates Mall).

We are currently seeking applicants for our full-time Infection Control/Employee Health Coordinator.

This position is full-time days

JOB/REQUIREMENTS/EDUCATION: Registered Nurse License, a minimum of 2 years RN experience. Completion of a NYS approved Infection Control Certification that is maintained or be willing to obtain. Must demonstrate excellent communication skills. Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing quality care.

JOB SUMMARY: Under the direction of the Director of Nursing, is responsible for assessing, coordinating, developing, and directing the Infection Control and Employee Health Programs for the facility in accordance with State and Federal regulations.

DUTIES:

Infection Control/Employee Health Program:

Directs infection prevention and control program through the collection and analysis of infection data; product/procedure evaluation; consultation on infection risk assessment, prevention and control strategies, education efforts directed at interventions to reduce infection risks; implementation of changes mandated by regulatory and licensing agencies (i.e. Antibiotic Stewardship); application of epidemiological principles to include activities directed at improving resident outcomes and provision of high quality services in a cost-efficient manner.

Acts as a resource for infection prevention and control in the facility.

  • Provides education for new employees on infection prevention and control practices in long term care and blood borne pathogens.
  • Provides in-servicing at annual mandatory education on infection prevention and control practices and blood borne pathogens for employees.
  • Ensures that nursing personnel follow established infection prevention and control practices and procedures through regular rounds on nursing units.
  • Ensures that ancillary personnel follow established infection prevention and control practices and procedures through regular rounds in all areas of the facility.
  • Prepares required infection prevention and control reports. Reports as necessary to DOH.
  • Maintains facility-wide surveillance as determined by QAPI Committee and the Medical Director.
  • Establishes and monitors performance indicators in infection prevention and control for QAPI Committee.
  • Provides education for residents and families on blood borne pathogens, infection prevention and control and tuberculosis as needed.
  • Maintains current infection prevention and control knowledge and practice through educational opportunities and quarterly attendance at meetings for the Association for Infection Control and Epidemiology (APIC).
  • Annually reviews and updates exposure control manual.
  • Monitors the incidence of residents and staff with communicable diseases such as tuberculosis or chickenpox.
  • Reviews and updates facility infection control and employee health policies and procedures.
  • Applies and reads PPD skin tests when needed.’
  • Administers Influenza, Hepatitis B, and MMR vaccinations when needed.
  • In conjunction with Nursing Administration, assists to direct resident immunization program for influenza and pneumonia.
  • Works with contractors and the facilities maintenance department to ensure that construction debris does not infiltrate residents care areas.
  • Provides routine consultations to staff, residents and families on biohazards and infection control.
  • Participates as a member of the QAPI Committee, Safety Committee and all others assigned.

PHYSICAL REQUIREMENTS: Normal office; lifting up to 20 lbs. as part of office activities.

PERSONAL REQUIREMENTS: The ability to read, write, speak and understand English; good communication and people skills; to work as a member of a team
