JOB SUMMARY: Responsible for providing On Call nursing care to hospice patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care community.
REPORTS TO: Director of Nursing
QUALIFICATIONS:
- A Registered Nurse professionally licensed to practice in this state who has at least one year experience in a hospice or home health setting.
- Current CPR certification
- Valid driver’s license, personal vehicle for on-the-job transportation, and evidence of insurance coverage as required by company policy.
- Ability to develop and implement improved methods of operation.
- Ability to supervise objectively without personal bias towards individuals and identify and evaluates personal strengths and weaknesses of self and others.
- Ability to solve practical problems; variety of variables with limited standardization; interprets instructions.
- Ability to communicate well, both verbally and in writing
- Ability to perform activities such as viewing a computer terminal, reading data and figures, visual inspection of patients.
- Required to have visual acuity to operate motor vehicles.
JOB CONDITIONS AND WORK DEMANDS:
- Requires minimal lifting of 50 lbs occasionally and up to 10 pounds of force constantly to move objects.
- Prolonged periods of sitting with periodic kneeling, standing, stooping, pushing, pulling and walking, as well as communication by phone and in person.
- Travel will be required, by car.
- Both inside and outside environmental conditions.
- Work Monday through Friday may include on-call work weekends, evenings, and nights.
ENVIRONMENT CONDITIONS AND RISK:
Frequently– Exposure to blood, body tissues, and other potentially infectious fluids
Rarely – Exposure to other hazardous waste materials
Frequently – Exposure to unpleasant odors as related to care of incontinent residents or wounds
Frequently – Exposure to latex, plastic and/or materials which are used for personal protective equipment
Occasionally – Exposure to marked changes in temperature and/or humidity
Frequently – Exposure to microbial bacteria and other infectious agents inherent to care of ill patient
Frequently – Exposure to fumes, odors, dust, poor ventilation, or smoking
RESPONSIBILITIES: (Included but not limited to):
- Assess home care and physical needs of the patient.
- Provide direct nurse care to patients as designated in care plan.
- Assume primary responsibility of care after business hours on assigned shifts.
- Document visit record electronically within 24 hours of the visit.
- Communicate with the on-call physician regarding patient change in status and pain and symptom control.
- Document all care provided on the patient’s clinical record in accordance with Hospice’s documentation requirement.
- Administer medications and other treatments in accordance with the physicians ' orders, generally accepted standards of practice, and any federal and state laws pertaining to medication administration.
- Teach and counsel patients/families on care giving techniques.
- Coordinate ongoing patient care with the interdisciplinary team and care partners.
- Ensure effective use of resources.
- Assures that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality.
- Serves as an educational source to staff, care Partners and the community.
- Ensure implementation of strategic goals.
- Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
- Participate in Hospice’s success by ensuring access to our exceptional care.
- Participate in the organization’s quality assessment and performance improvement
- Support and promote Hospice’s mission to lead in providing exceptional patient centered end-of-life care and grief support to anyone affected by a terminal illness.
- Adheres to all Agency, state and federal policies and procedures, laws, and regulations which are relevant to job responsibilities.
- Perform other duties as assigned.
