Admissions Coordinator – Skilled Nursing Facility

Location: Melbourne, FL

A well-established skilled nursing facility in the Volusia area is seeking an experienced Admissions Coordinator to manage referrals, admissions, and communication with hospitals, families, and internal clinical teams. This role is central to maintaining census and ensuring smooth, compliant admissions.

Key Responsibilities:

• Manage all incoming referrals from hospitals, case managers, and other referral sources
• Review clinical information and coordinate admissions with nursing and leadership teams
• Communicate clearly with patients and families regarding placement, paperwork, and next steps
• Verify insurance coverage and work with billing and business office teams as needed
• Ensure admissions are compliant with facility policies and payer requirements
• Maintain accurate admission records and referral tracking
• Support census development and conversion efforts

Qualifications:

• Prior experience as an Admissions Coordinator, Admissions Director, or Intake Coordinator in a skilled nursing or post-acute setting
• Strong understanding of SNF admissions, discharge planning, and payer sources (Medicare, managed care, Medicaid)
• Excellent communication and organizational skills
• Ability to manage multiple referrals in a fast-paced environment
• EMR and admissions software experience preferred
• Clinical background (RN, LPN, or social services) is a plus, but not required
