About The Role
The Social Health Intensive Care Coordinator will effectively assess, and manage clients impacted by mental health issues who are referred to Social Health Commonwealth Psychosocial Support Program (CPSP). The role will assess and support the needs of families and individuals by operating within a strengths and priority-based framework. Social Health Intensive Care Coordinator will deliver case management support for clients, providing a range of assessed social and wellbeing supports to clients in meeting their holistic recovery needs. Supports will be goal driven and focused on building the resiliency and capacity of individuals.
Duties
- Provide effective engagement with families and individuals in a welcoming manner, assess/screen their suitability for the CPSP program.
Ensure client’s risk assessments and safety plans are completed and accurate in progress notes and the IUIH Social Health Care Pathway.
Demonstrate the ability to manage Clients during a crisis and de-escalate towards securing sustainable and long-term assistance.
Continually engage with allocated clinic(s) to support medical staff in identifying referrals and managing social-emotional wellbeing issues.
Work as part of a multi-disciplinary team including Psychologists, Counsellors, GPs, Aboriginal Health Workers and Allied Health staff.
Utilise MMEx to accept referrals, document support and care, and communicate with other care providers within ATSICHS.

Base salary of $93,613 per annum (based on qualifications and experience).
12% Super + access to salary packaging (Increases your take home pay).
Fixed Full Time 6 months contract.

Immunisation history showing you have received two doses of a Covid -19 vaccine is encouraged.
Skills and Experiences:
Required
Certificate III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care.
National Police Certificate.
Working with Children's Blue Card and Working with Disability Yellow card or willing to apply.
Driver’s License.
Evidence of Work Rights.
Travel between work sites on a regular basis.
Travel to work in the community on a regular basis.
Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.
Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.
Ability to work with diverse people and presentations in providing case managed SEWB supports.
Demonstrated ability to record accurate and consistent case notes.
Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
