About the Role:

We are seeking a compassionate and skilled Social Worker to join our interprofessional healthcare team. The Social Worker provides comprehensive psychosocial assessment, counselling, education, and advocacy to support patients and their families in navigating the impacts of illness and hospitalization. This role focuses on enhancing patient and family well-being, facilitating safe and effective discharge planning, and connecting individuals to appropriate community resources. The Social Worker also collaborates closely with healthcare providers to address the psychosocial aspects of care while contributing to quality improvement and professional development initiatives.

What you’ll be doing:

  • Deliver psychosocial assessments and provide counselling, emotional support, crisis intervention, and grief support to patients and families.
  • Support comprehensive psychosocial assessments at the time of intake/referral and direct patients to the most appropriate outpatient support services
  • Support patients and caregivers in coping with the effects of illness and treatment, including family, financial, and social challenges.
  • Facilitate and coordinate individual and group counseling and connect patients and families with appropriate community resources and services.
  • Collaborate with physicians, nurses, and other interprofessional team members to ensure coordinated, patient-centered care.
  • Advocate on behalf of patients and families within the healthcare system and community.
  • In collaboration with a multidisciplinary planning committee, take an active role the development and implementation of the Ottawa Model for Smoking Cessation program
  • Provide consultation and education to healthcare teams regarding psychosocial issues and available resources.
  • Maintain accurate documentation and statistics in compliance with hospital standards.
  • Participate in program development, hospital committees, and quality improvement initiatives.

What you bring:

  • Master’s degree in social work from an accredited university.
  • Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Membership in the Ontario Association of Social Workers (OASW) is preferred.
  • Three (3)– five (5) years of relevant social work experience working with seriously mentally ill clients.
  • Advanced counselling and assessment skills with various short-term counselling modalities, including Brief Solution focused model, cognitive behavioural and dialectical behavioural therapy
  • Demonstrated ability to work collaboratively in an inter-professional team environment as well as independently.
  • Role models both professional and the organizational core competencies.
  • Ability to de-escalate a crisis situation
  • Good Working knowledge of Social Assistance forms
  • Knowledge of clinical practices
  • Knowledge of hospital informational systems, B Care, HPF, RM&R, Microsoft Office
  • Advanced group facilitation skills
  • Advanced assessment skills
  • Knowledge of community resources, income support programs and housing programs
  • Ability to work with individuals and their families
  • Advanced individual and group Counseling Skills, i.e. CBT, Solution Focused, Suicide Prevention Skills SMG Training
  • Understanding of the Mental Health Act and Mental Health Disorder
  • Training or experience in grief and loss required.
  • Strong knowledge of psychosocial assessment, counselling, and conflict resolution.
  • Demonstrated ability to collaborate effectively with patients, families, healthcare teams, and community partners.
  • Excellent organizational, problem-solving, and communication skills.
  • Flexibility and ability to work under pressure in a fast-paced healthcare environment.
