Social Worker – Integrated Patient Care Program

As a key member of the team, you will work closely with primary care providers and an interdisciplinary team to deliver patient-centered care that helps people stay independent and live healthier, fuller lives at home. The Social Worker works in the home care setting, in person and virtual visits, as part of the CBI Home Health team serving the Guelph - Wellington community.

What you need to be successful

  • You have a Bachelor of Social Work or a Master of Social Work
  • You are registered as a member in good standing with the Ontario College of Social Workers and Social Service Workers
  • You have the ability to travel within the Guelph - Wellington region community - Valid driver's license, vehicle and vehicle insurance required - East Wellington Region including Rockwood, Mapleton, Drayton, Minto, Mount Forest
  • Your focus is to embrace person and family-centered care
  • You are passionate, empathetic, use good judgement and apply critical thinking when making client decisions
  • You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
  • You value the role a therapist plays in keeping clients at home when that is of personal importance
