Social Worker - Msw - Case Management

Key Responsibilities:

As a Social Worker in this role, you will:

• Provide psychotherapy, counseling, and other essential services in collaboration with multidisciplinary teams.
• Operate under the direct supervision of a qualified social worker who meets state-required standards.
• Coordinate comprehensive patient discharge planning to ensure seamless transitions.
• Ensure compliance with all required medical and legal standards.
• Serve as a valuable resource, sharing your expertise and insights with allied health and administrative staff within the Social Work section.

Master of Social Work (MSW) degree from an accredited program.

Additional Qualifications

• Advanced knowledge of mental illness categories, personality development, and types of psychiatric treatment.
• Skills in conducting individual, group, and family therapy.
• Knowledge of adjustment to medical diagnoses and chronic diseases.
• Strong human relations skills with the ability to effectively interact with patients, patients' family members, consultants, residents, hospital personnel, community agencies, and other resources.
• Psychosocial diagnostic skills.
• Effective communication skills, both verbal and written.
