Position Description

POSITION SUMMARY:
To provide quality social work services congruent with the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers. Reporting to the Manager of the Schizophrenia Outpatient Clinic, the registered social worker will implement a full scope of practice in clinical care areas including: psychosocial assessment, counselling, family engagement, resource identification, facilitation of therapeutic groups, coordination and/or utilization of available resources, discharge planning, education, advocacy, professional development, and program planning. Practice will focus on individuals with serious and persistent mental illness using a recovery oriented model of care.

QUALIFICATIONS:
  • Clinical MSW and/or BSW from an accredited university, MSW is preferred
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Membership with the Ontario Association of Social Workers an asset
  • Minimum of two years mental health experience, specifically working with individual with schizophrenia or related psychotic disorders
  • Demonstrated experience completing comprehensive psychosocial assessment of individuals and families. Demonstrated experience with family engagement required.
  • Demonstrated expertise in Mental Status Examination and suicide risk assessment.
  • Demonstrated experience utilizing specific counseling modalities in individual, family and group therapies. Certification in CBT as well as well-developed skills in DBT, motivational interviewing, trauma informed care, addiction and concurrent issues required
  • Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families
  • Gentle Persuasive Approach training preferred
  • Comprehensive knowledge in mental health diagnoses and related rehabilitation treatments including familiarity with psychotropic medications, including potential side effects required
  • Ability to apply evidence based methods and relevant best practices for individuals with schizophrenia or related psychotic disorders required
  • Comprehensive knowledge in relevant regional community resources for effective and efficient discharge planning required
  • Superior skills in advocacy to negotiate systemic challenges and remove patient access systemic barriers required, including comprehensive knowledge of ALC process
  • Demonstrated ability to work and problem solve with minimal supervision required
  • Strong interpersonal skills to function as a productive member of an interprofessional team; to remain accountable to other team members and demonstrated adherence to relevant timelines is essential
  • Working knowledge of the relevant legislation, including the Mental Health Act, Health Care Consent Act, Privacy and Substitute Decisions Act essential
  • Ability to organize and prioritize multiple and competing demands required
  • Accurate and timely documentation, meeting professional, organization and service standards
  • Demonstrated commitment to own professional development and ongoing education
  • Interest and demonstrated abilities in program planning , development and research preferred
  • Good work performance and attendance record are essential
