Full time, 8:30 am - 4:00pm with on call rotation. Traveling position in the community.

Spiritual Care Counselors provide guidance for the spiritual and emotional needs of patients, families, and staff. Participate in and conducts worship, including various ceremonies/rituals, during visits to new patients and follow up visits, as requested. Works as part of an interdisciplinary team of care givers within HBI/CHPC network.

Essential Duties and Responsibilities for a Spiritual Care Counselor:

Assess and provide for patient spiritual care needs, as well as those of primary care givers and other family members as appropriate in compliance with all organization, regulatory, and professional standards.

Ability to build rapport with patients and family members, utilizing effective communication skills with openness toward different lifestyles, cultures, beliefs, and values; and understand the value of being a non-judgmental, listening presence.

Collaborates with interdisciplinary groups to arrange and successfully implement patient driven plans of care (with particular responsibility for spiritual care needs.)

Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative). Adheres to HIPAA regulations and policies.

Effectively and accurately document ongoing patient information within provided electronic medical record, according to organizational and professional standards-and maintains their security

Documents to agency standards inclusive of timely completion, timely signatures, and documents care using A-I-R protocol.

Participate in on-going education and professional development to maintain and acquire current expertise in the field of spiritual end-of-life care

Actively participate in team meetings, committees, and quality improvement initiatives as necessary

Serve as a spiritual care resource and presence to Hospice staff and volunteers as related to patient care issues

Facilitate involvement with local communities as necessary

Participate in organization's memorial services, speaking events, and gatherings as needed


Qualifications for a Spiritual Care Counselor:

Education/Certification

Master of Divinity, Pastoral Studies, Religion, or Theology or comparable field preferred.

Successful completion of 1 unit of Clinical Pastoral Education (CPE) within 2 years of hire.

Completion of multiple units preferred.

Board Certified Chaplain (or eligible for board certification) status preferred.

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

Experience

Two (2) years of experience working in spiritual or pastoral care, specifically with terminally ill patients
