Targeted Case Manager – Substance Use Disorder and Behavioral Health
Mountain Comprehensive Care Center is seeking a Targeted Case Manager to provide care coordination and support services for individuals receiving mental health and/or substance use disorder services. This role helps clients access resources, build stability, improve functioning, and work toward individualized recovery and treatment goals.
Targeted Case Management focuses on identifying client needs, coordinating care across systems, linking clients to community supports, monitoring progress, and documenting services clearly and consistently.
Position Overview
The Targeted Case Manager will work with clients to assess needs, develop person-centered care plans, coordinate services, and connect clients with appropriate resources. This position requires strong communication, organization, documentation skills, and the ability to work collaboratively with clients, families, providers, and other community partners.
Key Responsibilities
The Targeted Case Manager will:
- Complete and document comprehensive client histories, including support needs, skill deficits, level of functioning, risk status, and service needs.
- Assess client strengths, barriers, and priorities to guide service planning.
- Gather collateral information from medical providers, schools, social service agencies, family members, and other natural supports when appropriate.
- Develop person-centered care plans that reflect client goals, preferences, needs, and measurable objectives.
- Coordinate services and link clients with appropriate community resources.
- Assist with prior authorization requests and submit required documentation to support service approval and continuation.
- Document all direct and indirect contacts, including progress toward care plan goals.
- Communicate relevant updates, risks, and concerns with clinical team members to support continuity of care.
- Monitor implementation of the care plan and collaborate with supervisors regarding updates, reviews, and changes in client needs.
- Participate in care plan meetings and interdisciplinary team discussions.
- Maintain an active caseload of approximately 25 clients.
- Ensure timely and accurate documentation in compliance with agency, payer, and regulatory standards.
- Complete additional duties as assigned.
Qualifications
Minimum: Bachelor’s degree in human service field, (i.e. Psychology, Social Work, Counseling) with a minimum of one year experience. Minimum of one year Case Management experience required or a Master’s in human service may substitute.
