Sud Outreach Case Manager

Center for Human Services (CHS) is seeking a full-time, skilled, organized and caring outreach case manager with experience working with unhoused individuals with substance abuse and mental health issues. This position involves working as part of an interdisciplinary outreach team in the community. This is a full-time position.

POSITION SUMMARY:Requires in the field substance abuse screening, case management, and referral. Embedded and co-located member of Community Assessment, Response & Engagement (CARE) Multidisciplinary Team (MDT).

DUTIES:

  • Conduct outreach and engagement activities in the community.
  • Meet individually with clients regularly previous to substance use assessment to develop trust, educate, discuss substance use and consequences, discuss treatment options, information and resources, and motivate pre-contemplative clients toward treatment for SUD.
  • Provide crisis intervention services as indicated by client need during outreach and engagement, recognizing and intervening appropriately given signs and symptoms of use and withdrawal and/or mental health decompensation.
  • Complete substance use screenings of adults and arrange for substance use disorder services.
  • Provide case management, including locating and transporting clients to appointments, arranging emergency housing and recovery residence, picking up medications and other ancillary services to support getting client in to treatment.
  • Collect and organize program data.
  • Maintain case records in compliance with contractual, accreditation, professional ethics and licensing requirements.
  • Make daily decisions in assessing case needs, planning, and facilitating treatment and make joint service team decisions.
  • Utilize motivational interviewing, stages of change theory, and state-wise treatment strategies to intervene and motivate clients for change.
  • Collaborate with fellow CARE MDT members.
  • Maintain productive relationships with community partners, including Modesto Police Department Community Health and Assistance Team (CHAT).

QUALIFICATIONS:

  • High school diploma/GED required; BA/BS preferred.
  • Experience working with electronic health record or similar database system.
  • Experience and ability to work effectively with people of all socioeconomic and cultural backgrounds.
  • Effective communication, problem-solving, critical thinking, and interpersonal skills.
  • Case management or coaching experience preferred.
  • Understanding issues specific to substance use disorders.
  • Cultural sensitivity awareness and knowledge.
  • Knowledge of community resources.
  • Sound personal qualifications (i.e., maturity, stability, high moral standards and sound mental health.
  • Valid First Aid and CPR/Pro/AED, re-certification, as necessary.
  • Pass agency paid criminal justice screening including fingerprints.
  • Pass agency paid TB (Tuberculosis) screening.
  • Pass agency paid health screening and/or drug testing, if required.
  • Valid California drivers license and proof of auto insurance coverage.
  • Pass agency paid MVR clearance with no more than 2 points - if you are under the age of 25 our insurance company will not allow any points

  • Participate in assigned scheduled agency meetings, school meetings, in-service trainings, conferences and other trainings as determined by the supervisor. This includes serving as an agency representative at assigned community meetings.
  • Observance of assigned working hours and program appointments by demonstrating promptness and thorough preparation.
  • Performance of assigned duties with a positive attitude and in the spirit of teamwork, collaboration and cooperation.
  • Communicate effectively both orally and in writing.

  • Participates in and/or supports agency fund development activities and events.
  • Preparation of assigned reports, work records, statistical data, job performance evaluations, work plans, etc. in a timely manner.
