Description:
The Role
We have an exciting opportunity for a Support Coordinator to join our Client and Vision Services team based in Kew. This is a permanent role, working in a Full-Time/Part-Time capacity.
Reporting to the Support Coordination Team Leader, you will support clients who hold funding packages within the National Disability Insurance Scheme to plan the use of their funding and implement their services, and to assist clients who need support to access the National Disability Insurance Scheme.
Responsibilities
Support Coordination
Support the client to understand their NDIS Plan and assist them to develop an action plan in line with their identified goals.
Work with the client to implement funded formal supports, informal, mainstream and community supports in line with identified actions within a client’s National Disability Insurance Scheme plan.
Strengthen and enhance the client’s abilities to coordinate supports and participate in the community.
Ensure that mainstream services meet their obligations (such as housing, education, justice, and health).
Build the capacity of the client to achieve greater independence to self-direct service and supports in the longer term.
Provide the National Disability Insurance Agency with reports on outcomes and success indicators within the agreed reporting frequency.
Ensure funded supports are being implemented and used within allocated budget.
Coordinate referrals to service providers, including Guide Dogs Victoria, as requested by the client.
Ensure all client documentation including NDIS Service Agreement is completed and signed.
Evaluate progression against the client’s plan goals and assess the impact the services and supports have had on their life.
Support clients with NDIS applications and plan development.
Respect the way the client identifies including their culture, gender, sexual orientation and social community or communities.
Communicate effectively with clients in a way that is clear, inclusive and respectful.
Work collaboratively with the client’s support team which may include family, friends, advocates, or others nominated by the client.
Keep the client informed when sharing or discussing information about them to their support team.
Position Administration
Actively participate in the development and implementation of departmental objectives and complete other duties when requested to ensure their achievement.
Direct and supervise any volunteers within area of control, ensuring adherence to safe work practices at all times.
Provide accurate written information including accessible, clear and informative reports and communications.
Participate actively in staff meetings and share information to improve the work environment and outcomes.
Demonstrate initiative and contribute to ideas for improving ways of working and generating ideas for innovation.
Keep abreast of new developments within area of responsibility
Attend after hours Guide Dog Victoria events and activities as required from time to time.
Compliance
Follow all Guide Dogs Victoria policies, procedures and systems.
Ensure safety of self and others in the work environment.
Contribute to the identification and reporting of risks and hazards, and contribute to their effective control.
Ensure work meets Guide Dogs Victoria quality requirements, and contribute to the enhancement and continuous improvement of our quality systems.
Understand reporting obligations and how and when to report a Client incident or concern.
Maintain a satisfactory background check including a National Police Check, International Police Check (where relevant), National Disability Worker Screening Check, and Working with Children Check.
Skills and Experiences:
Qualifications
Certificate IV in Disability or Community Services or a relevant degree in Social Sciences
Knowledge, Skills and Experience
Solid understanding of the principles of the National Disability Insurance Scheme and how they apply in practice.
Strong communication skills to convey and explore deeply personal or complex issues in ways that are sensitive and respectful.
Solid skills and understanding of communicating in plain English.
Knowledge in working with people with individual funding.
Demonstrated skills in navigating the multiple sectors and linking individuals to relevant services and supports.
Has a good working knowledge of available supports and services of Guide Dogs Victoria as well as other complimentary service.
Excellent skills in a variety of Microsoft packages and databases, including Word, Publisher, Excel, Access and PowerPoint.
Experience in using CRM systems.
Ability to deal confidently and courteously with people at all levels both internally and externally to the organisation.
Excellent communication, telephone manner and interpersonal skills, and the ability to communicate sensitively and respectfully with all relevant stakeholders.
