Description:
Position Overview

Lead the transfer of community care services from Yarra City Council to YourCH. Shape inclusive, client-focused support by collaborating with partners, applying co-design, managing risks and improving service delivery across the community care network. This 7-month fixed-term, part-time role (0.5 FTE) reporting to the Community Programs Manager.

Lead HACC-PYP service shift into YourCH’s care model
Work with teams and partners to shape service delivery
Represent YourCH at key forums and networks
Run working groups to drive system change
Co-design improvements with clients and community

To succeed in this role you will have:
Qualification in health, disability, aged care or related field
5+ years' experience in health, aged care or community services
Proven project leadership and project management skills
Skilled in building and strengthening partnerships
Experience using co-design in planning and service delivery
