Job Summary
We are seeking a dedicated and detail-oriented Veteran Community Connection Specialist to join our team. The ideal candidate will be responsible for working with veterans experiencing homelessness in the Fredericton and surrounding areas. This role requires strong organizational skills, effective communication, in-depth case management and a thorough understanding of resources available for veterans.
Duties
- Work with community partners to receive referrals for veterans experiencing homelessness.
- Complete a prevention and diversion assessment with each veteran to determine if there is a possibility to divert to natural supports with resources.
- Connect veterans with Veterans Affairs Canada and any other resources that would be beneficial to the client.
- Prepare and maintain accurate documentation relating to the client, their housing needs and overall needs.
- Work with each veteran one-on-one through case management to move the individual forward in their journey.
- Work within our shelters as well as in community.
- Assist the client with searching for, and obtaining housing.
- Collaborate with other programs, staff and community partners.
- Document all necessary information on HIFIS and complete necessary reports.
Requirements
- Proven experience in case management.
- Strong knowledge of resources for veterans or individuals experiencing homelessness.
- Excellent organizational skills with the ability to manage multiple tasks simultaneously.
- Effective communication skills, both verbal and written.
- Ability to work independently as well as part of a team.

Pay: $20.00 per hour
Expected hours: 40 per week
