Wound Care LPN – In Home Wound Care
Personic Health is seeking LPNs, Paramedics, and other non-licensed healthcare workers to become certified Wound Care LPNs through our specialized training program. This is a mobile position, requiring individuals to have their own transportation to travel to patients’ homes and assisted living facilities. Training will be provided and paid for by Personic Health, with course fees covered.
Key Responsibilities:
- Perform assigned tasks under the direction of a licensed clinical staff member of a wound care team
- Utilize specialized knowledge, training, and skills acquired through our training program based on clinical research and wound management principles
- Work under clinical supervision/delegation of a medical provider or licensed nurse
- Perform duties according to prescribing medical provider orders, focusing on patient safety, and adhering to legal guidelines and regulations
- Conduct ABI assessments
- Facilitate Ultramist Ultrasound Therapy
- Carry out wound swabs
- This role requires daily and regular travel required to support patient-centric care both in-home and facilities
Qualifications:
- Proficiency in wound care
- Knowledge of safety and emergency procedures
- Basic patient care skills
- Understanding of infection control
- Excellent communication and interpersonal skills
- Ability to care for cognitively impaired clients
- Competence in focused assessment
- Ability to carry out delegated tasks and report appropriately
- Management of wound care materials and supplies
- LPN licensure within the state of vacancy
- Reliable transportation and the willingness to travel as directed to support the needs of the specified market
- Prior experience with Electronic Medical Records (EMR) systems
- An ability to capture digital consents when required

Standard Shift:
· 40 hours per weekday
· Monday to Friday, weekends as needed

· On the road, including Home Care, Long-term care facilities, Nursing homes, & Outpatient settings

Pay: From $70,000.00 per year
