About the Role:
We are seeking a dedicated Wraparound Care Coordinator to join our team. The Coordinator will work closely with children and families enrolled in the Children with Serious Emotional Disorder Waiver (CSEDW) program. You will be responsible for developing and managing personalized care plans (POCs), coordinating with service providers, and ensuring the delivery of high-quality services that meet the unique needs of each child.
Key Responsibilities:
- Develop and oversee individualized care plans (POCs) to support children and families.
- Lead Child and Family Teams (CFTs) to identify strengths, needs, and available resources.
- Conduct CANS assessments (a trauma-informed behavioral health evaluation tool).
- Ensure seamless coordination of services between families, providers, and community resources.
- Conduct regular in-home visits to assess progress, verify services, and address concerns.
- Assist with discharge planning to support a smooth transition for families.
- Maintain detailed case documentation for compliance and quality assurance.
Requirements:
- Bachelor’s degree in human services or a related field.
- 2+ years post-college experience in human services.
- Certification in CANS assessment & training in Wraparound Facilitation.(Training provided)
- Knowledge of community resources & experience in inter-agency coordination.
- Ability to work independently with strong communication and organizational skills.
- Certified provider under WV Medicaid & CSEDW.
- Must have wifi access and secure personal computer.
Preferred Qualifications:
- Experience working with children and families in mental health or social services.
- Prior involvement with CSEDW or similar programs.
