About the Aboriginal Health Access and Support Worker role

This First Nations role supports Aboriginal and Torres Strait Islander communities by connecting clients to culturally safe health services, coordinating care and advocating needs. It involves assessing clients, facilitating referrals, delivering health education and building strong community partnerships. Reporting to the Aboriginal Health Manager, this is a ongoing full-time role (1.0 FTE). Offices located in the Northern Suburbs.

Your Passion and Commitment will see you:
Support Aboriginal and Torres Strait Islander clients, carers and families to access culturally safe care
Assess needs and refer Aboriginal and Torres Strait Islander clients to services
Coordinate Case Conferences to support Aboriginal and Torres Strait Islander clients
Identify and address barriers affecting Aboriginal and Torres Strait Islander access to care
Build strong relationships with Aboriginal and Torres Strait Islander communities and organisation

To succeed in this role you will have:
An Aboriginal and/or Torres Strait Islander person is sought for this role by Your Community Health as a special measure pursuant to Section 12 of the Equal Opportunity Act 2010.

  • Relevant experience and/or qualifications such as Aboriginal Health Worker, Certificate III in Mental Health, Certificate III in Aged Care, Certificate IV in Community Services or willingness to work towards one or more of these qualifications.
  • Current Victorian Driver’s License
  • Experience working with Aboriginal and Torres Strait Islander communities
  • Strong understanding of cultural safety, awareness and sensitivity
  • Knowledge of social, cultural and health challenges impacting community
