Job Summary
The Care Coach is responsible for calling, supporting, and monitoring patients living with chronic conditions. You will build rapport, collect health information, document updates, and escalate concerns to our clinical team. This is meaningful work that directly improves patient outcomes between regular doctor visits.

This is a remote position requiring a quiet home office, strong internet, and a commitment to meeting quality and productivity expectations.

Responsibilities

We are looking for individuals who bring both a caring heart and a diligent work ethic.

• Call assigned patients each month to conduct care-management check-ins
• Build rapport and provide a positive, supportive patient experience
• Gather health updates and document changes in our system
• Follow scripts, workflows, and escalation protocols
• Identify concerns and route urgent issues to clinicians
• Assist patients with understanding their RPM/RTM programs
• Meet monthly productivity standards while maintaining high call quality
• Participate in team meetings and continuous improvement efforts

Qualifications

• Experience working with adults/elderly patients or individuals with chronic conditions
• Experience working independently from home
• Medical office, home health, care coordination, call center healthcare support, or related experience
• Strong communication and active-listening skills
• Ability to meet productivity targets without sacrificing quality
• Proficiency with computers and multiple software platforms
• Reliable high-speed internet
• Quiet, distraction-free workspace

Preferred:

• Background as a Medical Assistant, LPN/LVN, CNA, home health aide, or similar
• Experience communicating with patients who have chronic illnesses
• Strong documentation and typing skills
• Bilingual candidates encouraged to apply

Job Type: Full-time

Pay: $18.00 - $20.00 per hour

Expected hours: 40 per week
