We are seeking a passionate and skilled Care Navigator to join our Child and Adolescent Mental Health Service (CAMHS) and support young people with complex neurodevelopmental and mental health needs.
About the Opportunity
- Act as a key point of contact for families attending CAMHS, supporting them to navigate complex service systems.
- Collaborate with families and multidisciplinary teams to develop and implement timely, achievable, person-centred care plans.
- Coordinate and facilitate multidisciplinary team (MDT) processes, including collating information, organising meetings, and tracking agreed goals.
- Develop and maintain effective service pathways between CCLHD, NDIS, Early Childhood Early Intervention (ECEI), CMOs, NGOs, schools, and private providers.
- Provide consultation and liaison support to CCLHD staff regarding best practice, referral options, and available supports for neurodevelopmental and mental health needs.
- Contribute to service improvement projects, including the development of referral tools, service maps, and consumer-focused information resources.
Support system-level capability building by improving knowledge and confidence in working with neurodiversity, recovery-oriented practice, and NDIS processes.

- Occupational Therapist Level 3
- Physiotherapist Level 3

- Speech Pathologist Level 3
- Clinical Psychologist
- Senior Psychologist
About You
- Degree qualifications in your relevant clinical discipline and current professional registration (e.g. AHPRA or relevant professional body), with a minimum of 3 years post-qualification clinical experience.
- Demonstrated experience working within Child and Adolescent Mental Health Services or a mental health setting.
- Strong understanding of Recovery Oriented Mental Health Practice.
- Experience in case management and developing, implementing, and reviewing client-centred support plans.
- Proven ability to work independently and collaboratively within a multidisciplinary team.
- Experience working with community partners, including GPs, schools, NGOs, CMOs, and disability services.
- Current Working With Children Check, National Police Check, Category A immunisation compliance, and a current Australian Driver’s Licence.
