  • Conduct routine facility visits, clinical audits, mock surveys, and operational reviews to assess compliance with state, federal, and company standards.
  • Monitor key performance indicators, including quality measures, infection prevention, rehospitalization rates, staffing metrics, and survey outcomes.
  • Develop and implement corrective action plans and performance improvement initiatives where needed.
  • Serve as a mentor and resource to Directors of Nursing, Assistant Directors of Nursing, MDS Coordinators, and clinical leadership teams.
  • Assist with recruitment, onboarding, training, and development of nursing leadership personnel.
  • Ensure facilities maintain compliance with CMS, Connecticut Department of Public Health, OSHA, HIPAA, and other applicable regulations.
  • Lead and support survey readiness efforts and provide on-site assistance during state and federal surveys.
  • Collaborate with corporate leadership to implement clinical programs, policies, procedures, and best practices across all centers.
  • Support infection prevention, risk management, quality assurance, and resident safety initiatives.
  • Review clinical documentation, care planning processes, and reimbursement-related practices to ensure accuracy and compliance.
  • Analyze clinical trends and data to identify opportunities for improvement and implement evidence-based solutions.
  • Participate in leadership meetings and contribute to strategic planning initiatives that enhance resident care and organizational success.
  • Travel regularly between assigned facilities throughout Connecticut.

QUALIFICATIONS:

  • Current, unrestricted Registered Nurse (RN) license in Connecticut.
  • Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing, Healthcare Administration, or related field preferred.
  • Minimum of five (5) years of nursing leadership experience in Skilled Nursing Facilities (SNF) or Long-Term Care (LTC).
  • Previous experience as a Director of Nursing required.

  • Demonstrated success improving clinical outcomes, survey performance, and regulatory compliance.
  • Strong leadership, coaching, mentoring, and team-development skills.
  • Exceptional analytical, organizational, communication, and problem-solving abilities.
  • Ability to effectively manage multiple priorities and facilities simultaneously.
  • Proficiency with electronic health records and healthcare technology systems.
  • Ability to travel extensively throughout Connecticut.
  • Must be able to read, write, and understand English.
