Hospice Social Worker

Responsibilities:
- Provide counseling and support to individuals with autism, developmental disabilities, dementia, PTSD, and other mental health conditions.
- Conduct assessments to determine clients' needs and develop individualized treatment plans.
- Collaborate with other healthcare professionals to coordinate care and ensure holistic treatment.
- Assist clients in accessing community resources and support services.
- Advocate for clients' rights and ensure they receive appropriate care and services.
- Provide crisis intervention and manage emergency situations effectively.
- Maintain accurate and up-to-date documentation of client interactions and progress.

Experience:
- Minimum of [X] years of experience as a social worker in a healthcare or mental health setting.
- Experience working with individuals with autism, developmental disabilities, dementia, PTSD, and other mental health conditions.
- Knowledge of medical terminology and systems.
- Proficiency in conducting assessments and developing treatment plans.
- Strong crisis management skills.
- Ability to provide patient-centered care and support.

Qualifications:
- Bachelor's or Master's degree in Social Work or related field.
- State licensure or certification as a social worker (if applicable).
- Excellent communication and interpersonal skills.
- Compassionate and empathetic approach to working with individuals in need.
- Ability to work independently as well as part of a multidisciplinary team.
