About Us
The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
Are you a passionate and motivated Physiotherapist looking to make a meaningful difference in the lives of older people within your community? We have an exciting opportunity to join our dedicated and supportive team, delivering high-quality physiotherapy services across a range of community-based programs, including the Transitional Aged Care Program (TACP), Nepean Post Acute Community Care (NPACC), and the Commonwealth Home Support Program (CHSP).
In this rewarding role, you will work with clients transitioning through various stages of care, supporting them to regain independence, improve mobility, and enhance their overall quality of life. Your work will take place across community settings, including clients' homes and Community Health Centres, providing a diverse and engaging caseload. As part of a collaborative multidisciplinary team, you will have the opportunity to apply your clinical expertise, creativity, and compassion to deliver person-centred, evidence-based care that has a genuine impact.
We are seeking a qualified Physiotherapist with a strong interest in aged care, slow stream rehabilitation, and community health. You will bring excellent clinical reasoning skills, a commitment to delivering high-quality care, and the ability to build positive therapeutic relationships with clients, families, and colleagues. A proactive approach, strong communication skills, and a passion for helping people achieve their goals will see you thrive in this role.

- Tertiary qualifications in Physiotherapy and Current National Physiotherapy Registration with the Australian Health Practitioner Regulation Authority (AHPRA).
- Demonstrated ability to provide high-quality physiotherapy assessment and intervention across a broad range of clinical presentations, commensurate with level of experience.
- Demonstrated effective written and verbal communication skills.
- Demonstrated effective time management skills.
- Demonstrated commitment to professional development and demonstrated knowledge of Quality Improvement & Evidence Based Practice.
- Demonstrated high level communication and interpersonal skills within a health environment, with experience working in multidisciplinary team settings.
- Current Driver's Licence and willingness to use for work purpose travel. This may include driving long distances across the LHD.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
