The Role of the Primary Care Nurse is in the focus of maintaining and supporting LTSR patient health, behavioral, cognitive and spiritual goals.

Schedule: Tuesday-Saturday, 7am-330pm

· Consistently promote compliance to prescribed medical treatments, healthy lifestyle choices remembering to maintain a mind/body balance in accordance with the community supports program principles.

· Provision of direct services to residents, including assistance with personal hygiene and tasks of daily living as needed.

· Developed and track self-medication education goals for individuals in recovery.

· Facilitates/Co-Facilitates community education activities to enhance the knowledge, understanding and interest in each individual's own health status.

· Work directly with the psychiatrist by assuring verbal orders, paperwork and information is readily available to support the best quality of care is provided to the individuals in recovery.

· Facilitate case management by exploring symptoms, diagnosis and associated care teams assuring the individual in recovery is able to access all possible services available.

· Develop and train adherence to proper nutrition guidelines, individual dietary restrictions/special diets and dietary supplements, as indicated and prescribed.

· Completes Annual nursing assessments/Medical histories.

· Completes on-sight admission process; including post admission packet, medication orders and assuring durable medical equipment is available prior to admission.

· Participates in multi-disciplinary treatment team meetings weekly, communicating overall medical wellbeing and status on medication changes, side effects and compliance issues.

Current, unrestricted PA Licensed Nurse with 2 + years Mental Health or Case Management Experience. (LPN & RN welcome to apply)

Pay: From $62,000.00 per year

Work Location: In person
