Job Summary
We are seeking a highly skilled and compassionate Registered Nurse to join our healthcare team. We are a pediatric home health care agency working with the GAPP program.

The ideal candidate will possess extensive clinical experience across various specialties, including critical care, pediatrics, and home care settings. This role involves providing comprehensive patient care, performing assessments, administering medications and treatments, and collaborating with multidisciplinary teams to ensure optimal health outcomes. The nurse will also be responsible for discharge planning and case management in home care settings.

Responsibilities

• Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations.
• Administer medications via injections, IV infusions, and other routes following aseptic techniques and safety protocols.
• Perform specialized procedures such as tube feeding management.
• Document all patient interactions accurately using EMR/EHR systems.
• Collaborate with physicians and healthcare teams on diagnostic evaluation and treatment plans.
• Support patients with disabilities or developmental challenges.
• Maintain compliance with HIPAA regulations while managing medical records and documentation of intake data.

Experience

• Proven experience in diverse clinical environments such as hospital medicine (including ICU/CCU/PICU), outpatient clinics, long-term care facilities (nursing homes & assisted living), home & community care settings.
• Demonstrated proficiency with EMR/EHR systems.
• Familiarity with coding standards such as ICD-9/10 for accurate documentation and billing purposes.
• Experience working with pediatric populations.

This position is part time 8hrs a week or 30 hours a month which can lead to a full time opportunity starting in April.

January hours will be more than 8hrs a week because we have to fulfill the total monthly hours within the next 11 days.

Schedule is flexible, but preference given to an immediate hire. YOU MUST BE RELIABLE.

Specific patient dx CP, TBI, Gtube.

Pay: $50.00 per hour
