The Hospital in the Home (HiTH) service is a virtual ward designed to facilitate a smooth transition for patients with acute or sub-acute conditions who are deemed by a senior medical officer to be suitable for care outside of a traditional hospital ward. This is a fixed-term position to cover parental leave.

The HiTH service operates seven days a week, from 7 am to 10 pm, and is fully supported by a Consultant and RMO.

We are seeking a Registered Nurse to join our team. In this role, you will conduct comprehensive nursing assessments, identifying patient needs, risks, and early warning signs that impact care plans and required interventions.

Clinical health assessments will be conducted via telehealth, including health history, observations, risk assessments, diagnostic tests, as well as psychosocial, cultural, and learning needs. You will be expected to act proactively in managing patient care.
