Health NZ Counties Manukau is a dynamic and progressive organisation at the forefront of innovative health care delivery within the New Zealand Public Health Sector. We continue to work with others to achieve equity in key health indicators for Maaori, Pacific and communities with health disparities and will continue to develop and implement plans, based on local needs to improve the health and wellbeing of Counties Manukau region. Our main sites are Middlemore Hospital, Kidz First Hospital and Manukau Health Park.

We are part of Health New Zealand - Te Whatu Ora, the overarching organisation for New Zealand’s national health service. HNZ leads the day-to-day running of the health system, with functions delivered at local, district, regional and national levels.

About the role:

An exciting new position has become available for a highly motivated registered nurse, who will focus across Hospital & Specialist Services contributing toward achieving equitable outcomes for Maaori. Situated within Te Kaahui Ora – Maaori Health you will play a key role in to ensuring care pathways mitigate avoidable, unjust or unnecessary barriers caused by the system of health care for Maaori.

The aim is to ensure optimal care co-ordination for Maaori who have complex health and social needs, and to safely transition care to community and primary healthcare providers to ensure ongoing health and social needs are met. This will require engaging, facilitating, and working with patient and whaanau to achieve whaanau directed goals. Establishing and maintaining effective working relationships with members of the healthcare team across the continuum will be essential.

The role requires intermediate cultural and clinical knowledge, and applied reasoning to safely co-ordinate care for individual patients admitted to hospital, to safety transition care into the community and primary healthcare setting.

Essential:

  • Minimum three years of relevant experience post registration
  • Experience working with patients with complex health and social needs
  • Excellent written and verbal communication skills
  • Qualified Registered Nurse
  • Current NZ Practicing Certificate
  • Working towards Post-Graduate qualification
