Registered Nurse (Rn) - Home Health

Scarlet Home Health Care is seeking a part-time or per diem RN Case Manager to join our dynamic team! The ideal candidate will have a heart for working with home-bound patients, a passion for making a difference in their well-being, and the clinical expertise to manage complex care needs in a home setting.

The area of coverage for this position is Newark, Heath, and surrounding areas.

What You'll Do:

• Collaborate with physicians to address medication needs and make necessary adjustments to care plans.
• Lead and supervise other healthcare disciplines, including LPNs, aides, and therapy staff, to ensure that each patient receives the highest standard of care.
• Complete OASIS, recertifications, discharge documents, and other necessary documentation within the EMR system.
• Actively monitor and assess changes in patient conditions to help prevent re-hospitalization, ensuring proactive interventions.
• Provide education and counseling to patients' families about care processes, disease management, and injury recovery.
• Attend orientation sessions, team meetings, and Continuing Education Units (CEUs) to maintain professional growth.

Requirements:

• Current Ohio RN license in good standing.
• Current CPR certification.
• Reliable auto and auto insurance.
• Ability to work independently with minimal supervision.
• Solid understanding of Medicare/Medicaid guidelines in home health care.
