About You

Additional attributes we are looking for include:
  • Experience in assertive outreach, youth work, case management, mental health, or alcohol and other drugs (AOD) (professional, volunteer, or lived experience); or relevant qualifications in psychology, mental health, social work, or related fields.
  • Ability to quickly and effectively build rapport with individuals.
  • Demonstrates flexibility, patience, confidence, initiative, and emotional resilience.
  • Strong listening skills and adaptable communication style.
  • Ability to contribute positively to a team while also working independently to manage your caseload.
We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes, including (but not limited to) wearing appropriate PPE and regular hygiene and cleanliness practices.

About the Role
ermha365 provides the Early Intervention Psychosocial Support Response (EIPSR) program in partnership with Barwon Health. EIPSR provides specialist psychosocial support to help eligible participants to build their capacity to better manage their mental health, develop practical life skills for independent living and social connectedness, and if eligible, transition to the NDIS.

The Support Facilitator’s goal is to enable rather than to do; to promote independence, development and confidence in the participant.

The role entails:
  • Provide psychosocial support to young people and adults with severe mental illness on a short to medium term basis (up to 12 months).
  • Manage a caseload of 10-15 participants providing support with mental health & wellbeing goals
  • Collaborate with participants, clinicians, and other stakeholders (e.g., family) to access and engage with various support services.
  • Develop, monitor, and track recovery goals, implementing steps to promote independence.
  • Conduct screenings and assessments, and participate in case planning and exit reviews alongside care teams and clinical staff.
  • Advocate for individuals and build sustainable support networks.
This role works on a hybrid system, including working from home, in our Geelong office and assertive outreach support to participants across the Greater Geelong region and Bellarine Peninsular.
Program hours are typically 8am to 4pm (some flexibility). This is a contract position ending 30/06/2026.

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver’s licence.
