Position: Support Home Monitor
Location: Edmonton, AB
About Us: Edmonton Integrated Services (EIS) is dedicated to enhancing the quality of life for individuals with disabilities through comprehensive, person-centered support. We are currently seeking a dedicated and compassionate individual to join our team as a Support Home Monitor (CDSP) in our Supportive Living Program.
Position Summary: As a Support Home Monitor, you will play a vital role in ensuring the quality and effectiveness of residential and outreach supports. Your primary responsibility will be to oversee and monitor the day-to-day operations of our support homes, ensuring adherence to health and safety protocols and the implementation of individualized support plans.
Key Responsibilities:
- Regularly monitor support homes to ensure compliance with health and safety protocols.
- Collaborate with individuals and their support networks to develop and document personalized, person-centered plans.
- Facilitate the implementation of these support plans, ensuring they meet the needs and preferences of each individual.
- Ensure that services provided align with the support plans and agency requirements.
- Maintain clear and objective records, completing all necessary reports and documentation.
- Educate support/service providers on Agency Policies and Procedures and CET standards relevant to their roles.
Qualifications:
- Education: Post-secondary diploma or degree in Community Rehabilitation, Disability Studies, or a related field.
- Experience: Minimum 3 years of experience supporting individuals in a community setting, with some supervisory experience is an asset. Crisis Prevention and Intervention experience is required.
- Skills:
- Strong belief in the rights of persons with disabilities to define and pursue their life choices.
- In-depth understanding of opportunities and options available to persons with disabilities.
- Excellent communication skills and the ability to work effectively both independently and as part of a team.
- Ability to identify issues, set priorities, and implement appropriate actions.
- Proficiency in reporting and computer skills.
- Effective time-management and organizational skills.
Other Requirements:
- Reliable vehicle with personal liability insurance (minimum $2,000,000 coverage).
- Current Police Security Clearance.
