Job Summary
The Support Service Provider plays a crucial role in assisting families by providing guidance, resources, and support to enhance their overall well-being. This position involves working with a client in the Altoona area. The schedule will be Tuesdays and Thursdays, 7:30am to 4pm. The ideal candidate has experience in the intellectual/physical disability field. Experience with mobility/physical therapy a plus.
Responsibilities
- Conduct assessments to determine the needs of families and individuals.
- Implement individualized support plans tailored to each family's unique situation.
- Maintain accurate records of client interactions, services provided, and progress made.
- Collaborate with other service providers and community organizations to ensure comprehensive support for families.
- Ensure confidentiality and professionalism in all interactions with clients.
Qualifications
- Proficiency in computer applications, including Microsoft Office Suite for data entry and submissions.
- Strong client service skills with the ability to empathize and connect with families from various backgrounds.
- Excellent organizational skills with attention to detail for maintaining accurate records.

Job Type: Part-time
Pay: $21.00 - $27.00 per hour
