Support Service Provider job opportunity

Job Summary
The Support Service Provider plays a crucial role in assisting families by providing guidance, resources, and support to enhance their overall well-being. This position involves working with a client in the Altoona area. The schedule will be Tuesdays and Thursdays, 7:30am to 4pm. The ideal candidate has experience in the intellectual/physical disability field. Experience with mobility/physical therapy a plus.

Responsibilities

Conduct assessments to determine the needs of families and individuals.
Implement individualized support plans tailored to each family's unique situation.
Maintain accurate records of client interactions, services provided, and progress made.
Collaborate with other service providers and community organizations to ensure comprehensive support for families.
Ensure confidentiality and professionalism in all interactions with clients.

Qualifications

Proficiency in computer applications, including Microsoft Office Suite for data entry and submissions.
Strong client service skills with the ability to empathize and connect with families from various backgrounds.
Excellent organizational skills with attention to detail for maintaining accurate records.

Pay: $21.00 - $27.00 per hour
