Position Overview
The Care Coordinator is responsible for overseeing and coordinating all aspects of the Wellness Department to ensure the safe, effective, and high-quality delivery of resident care services. This role provides clinical leadership to the nursing and personal support team, ensures compliance with RHRA standards and internal policies, and fosters a culture of professionalism, accountability, and compassion.
Works collaboratively with the Director of Operations, physicians, families, and interdisciplinary team members to promote resident independence, dignity, and well-being.
Key Responsibilities
Leadership & Oversight
- Lead and supervise all nursing and personal support staff (RPNs, PSWs, and wellness aides).
- Provide ongoing coaching, mentoring, and performance management to ensure high standards of resident care.
- Prepare and manage department schedules and ensure adequate coverage 24/7.
- Support staff education, skills development, and adherence to best practices.
Resident Care Management
- Oversee the development, implementation, and evaluation of individualized resident care plans.
- Ensure all assessments and documentation are completed accurately and in a timely manner.
- Monitor medication administration processes and compliance with medication policies.
- Coordinate with physicians, pharmacists, and allied health professionals regarding resident care needs.
- Participate in care conferences, admissions, discharges, and resident/family meetings as required.
Regulatory Compliance & Risk Management
- Ensure compliance with the Retirement Homes Act, RHRA regulations, and internal policies.
- Maintain accurate and confidential resident health records.
- Conduct audits (care plans, MARs, incident reports, etc.) and follow up on corrective actions.
- Report and manage incidents, complaints, and infection control concerns promptly.
- Lead internal audits and readiness for RHRA inspections and Ministry reviews.
Collaboration & Communication
- Work collaboratively with all departments to promote holistic resident care (Culinary, Recreation, Environmental, etc.).
- Communicate professionally with residents, families, and external partners.
- Act as the primary clinical contact for resident health-related concerns.
- Participate in management meetings and contribute to operational improvement strategies.
Administrative Duties
- Oversee departmental budget and expenses; review invoices and payroll with the Controller.
- Maintain accurate records for staffing, training, and compliance.
- Prepare reports for the Director of Operations, RHRA, and other stakeholders as required.
- Lead monthly wellness meetings and staff training sessions.
Qualifications
- Current Certificate of Registration with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN) or Registered Practical Nurse (RPN).
- Minimum 3–5 years of leadership experience in a retirement, long-term care, or healthcare setting.
- Strong knowledge of RHRA regulations, Retirement Homes Act, infection prevention, and medication management standards.
- Proven leadership, organizational, and interpersonal communication skills.
- Ability to handle sensitive information with tact and confidentiality.
- Current CPR and First Aid certification.
- Clear Vulnerable Sector Police Check required prior to employment.

Job Type: Full-time
Pay: $70,000.00-$80,000.00 per year
