Job Description
Facilitate clinical case management duties that will ensure the best coordination of care for the clients. Facilitate and carry out medical professionals’ plan of care ordered by lead physicians. The case manager is seen as the expert in coordinating the plan of care and relatedness of services to injuries, knowledge of benefits, knowledge of community resources, knowledge of auto no-fault processes, and communication of information to appropriate people. The Care Coordinator is accountable in assuring all orders are facilitated, coordinated and become part of the client chart. This position will have a home base in Troy, MI with occasional travel to Utica, MI and client appointments.
Duties:
- Attend weekly meeting(s) in order to coordinate/relay client information to Directorship, clinicians, site managers, and administrative staff. Meetings include, but are not limited to; Operations, Utilization Review, Clinical, and Manager Meeting.
- Participate in monthly treatment team meetings. During meeting, record pertinent information along with updating external stakeholders in regards to clients progress with program goals, medications, and medical appointments.
- Attend Medical appointments with HCBR members.
- At medical appointments, update health care provider’s regarding client’s progress in programming, any concerns, and changes in medications.
- Update documentation weekly (Utilization Review, Monthly Report, Progress Note, and Outlook calendar) to reflect; medication updates, medical appointments time/dates, attendant care changes, participation in therapies, treatment team notes, stakeholder contacts.
- Attend Court Dates as needed.
- Draft reports as needed for court, probation department, etc.
- Assist in arranging medical appointments and transportation needs.
- Maintain HIPPA guidelines by obtaining and updating release of information forms annually.
- Coordinate community resources for treatment to assist in recovery from behavior challenges, secondary issues of cognitive, behavioral and emotional challenges due to dual diagnosis, following through to ensure service efficacy.
- Manage and coordinate with site supervisors and life skills staff on a daily basis in regards to client’s medical, transport, and daily needs.
- Act as an advocate and liaison for members to relay concerns/wants/needs to Communicare Staff, External stakeholders, and medical providers.
- Required to keep a company cell phone at all times (even outside of scheduled work hours) to receive phone calls from site/shift supervisors, on-call administrators, and Life Skills Specialists if necessary, and assist as needed.
- Provide emotional support to client’s and family members when adjusting to the changes that result from Acquired Brain Injury, dealing with court proceedings, and changes/discontinuation of benefits.
- Provide client with transportation to appointments, as needed.
- Refer client and/or family to community resources as needed, including but not limited to; financial assistance, legal aid, housing, job placement, and education.
- Communicate frequently with external case managers and other stakeholders.
- Ensure that weekly clinical therapy schedule meets treatment plan goals.
- Other duties as assigned
Working conditions:
- Work weeks are 5 days, Monday to Friday, 40 hours per week.
- Entitled to one half-hour of paid time for lunch.
- Frequently exposed to noise and a regular flow of people around the offices and job sites.
- Frequently assigned to changing priorities.
- Daily travel to various sites/client homes/medical appointments.
- Physical activity that often requires keyboarding, sitting, phone work, filing, driving.
- Physical activity that often requires lifting under 25 lbs.
- Physical activity that sometimes requires bending, stooping, reaching, climbing, kneeling and/or twisting.
Employment Requirements:
- Minimum of a Bachelors in Social Work, Nursing, or Human Services.
- No criminal record.
- Satisfactory driving record.
- Excellent written and verbal communication skills.
- Knowledge of governing law, standards of care for case management and insurance parameters of case management delivery.
