Introduction:

Description:
About the Role:
As a Case Manager at SwanCare, you will deliver care management activities that enable older people to live safely and independently in their own homes. You will collaborate with participants, their supporters and interdisciplinary teams to coordinate tailored, responsive and inclusive support.
