Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job Description
The opportunity:
We are looking for an Occupational Therapist to join our supportive and professional multidisciplinary teams in an ongoing, full-time capacity.
Responsibilities include but are not limited to:
- Deliver high-quality, client-centred Occupational Therapy services including assessment, goal setting, intervention, and assistive technology support across a diverse community caseload.
- Be part of a forward-thinking organisation that is exploring and embedding innovative technologies, including AI-enabled tools, to support clinical excellence, efficiency, and improved client outcomes.
- Build strong relationships with clients, families, carers, and multidisciplinary teams to achieve meaningful outcomes and support wellbeing.
- Manage a varied workload effectively, maintaining accurate documentation, reporting, and compliance with funding, professional, and organisational requirements.
- Contribute to a collaborative team culture through case discussions, supervision, quality improvement activities, and support of students and junior staff.
- Promote safe, ethical, and evidence-based practice while working across community health settings and healthAbility service locations.
Desired Skills and Experience
Our ideal candidate:
- Minimum 5 years' experience as an Occupational Therapist, ideally within community health or a related service setting.
- Excellent communication and relationship-building skills, with the ability to work collaboratively within multidisciplinary teams and with external service providers.
- Demonstrated ability to contribute to a culture of shared learning, collaboration, and problem-solving.
- Adaptable and proactive, with the ability to thrive in a dynamic and evolving environment.
